How to Submit a Claim

It’s good to know your DGC Benefits Plan provides valuable coverage, but to make the most of that coverage, you need to submit your claims — so you can be reimbursed quickly and efficiently.

Prescription Drug Claims

Present your drug card to your pharmacist when you fill your prescriptions. The pharmacist will process your drug claim on the spot. All you pay is the portion that is not covered by the plan.

Did you know? If you have money in your HSA, you can submit a claim to your HSA or use your HSP VISA card for the portion not covered by the plan.


If you forget or misplace your card

You can also submit your claim online or on paper, using the Healthcare Expenses Statement form.

 

You can print a paper copy of this form through My Canada Life at Work.

An easy way to avoid the stress of forgetting your drug card is to download the My Canada Life at Work app from Google Play or the Apple App store. With the app, you’ll have easy access to your digital benefits card. You can save this card to your phone to present at the pharmacy whenever you need to.

How to submit electronic claims

To submit an electronic claim, log in to your My Canada Life at Work account — either online (mycanadalifeatwork.com) or through the My Canada Life at Work app from Google Play or the Apple App store.

How to submit paper claims

If you prefer to use a paper claim form, you can print out a paper copy of the “Healthcare Expenses Statement” through My Canada Life at WorkThe health policy number is 50553. You will also need to provide your ID number (which is your DGC member number).

 

If you submit a paper form, attach all original receipts (or the original Explanation of Benefits if you have already submitted this claim under another ).

Plan number

When submitting paper claims, you will need to provide the plan number 50553 and your ID number (which is your DGC member number).

 

These numbers are on your DGC Benefits card from Canada Life. You can reprint this card any time from My Canada Life at Work.
